Our members are now able to begin the FTX challenge process online via our website. The form is available at smart528.org/ftx or under under the members tabs.
A lot of the information that we need to begin the process is the same information that is available on the MyUP website.

If more of our members learn how to save/print PDF files then in the future our office would allow the Claim Slips and Cut-Slips to be submitted online.
